• For treating dementia associated with Alzheimer’s Disease, the recommended starting dose is the 4.6 mg dose, every 24 hours. The dose may only be increased to 9.5 mg after four (4) weeks if the dose is well-tolerated. The dose may be increased to 13.3 mg after an additional four (4) weeks. 
• For mild to moderate Alzheimer’s Disease, the recommended dose range is 9.5 mg to 13.3 mg, every 24 hours. 
• For moderate to severe Alzheimer’s Disease, the recommended dose is 13.3 mg, every 24 hours. 
• For treating dementia associated with Parkinson’s Disease, the recommended starting dose is the 4.6 mg dose, every 24 hours. The dose may only be increased to 9.5 mg after four (4) weeks if the dose is well-tolerated. The dose may be increased to 13.3 mg after an additional four (4) weeks. 

If the dosage of the transdermal patch is interrupted for less than 3 days, restart treatment with the same or lower dosage of the patch. If the dosage of the transdermal patch is interrupted for more than 3 days, restart the treatment with the lowest dosage (4.6 mg). 

• For patients with renal or hepatic impairment, dosage adjustment may not be required. Consult your doctor for more information about your dosage.  
• For patients who weigh less than 50 kg, monitor toxicities and discuss dosage reduction. If toxicities develop with the transdermal patch, reduce the dosage to 4.6 mg, every 24 hours.
